Student Media Permission Form

The BUSY School Calgary may take photos, video or audio of you, or invite you to take part in an interview or provide a quote. This form lets you or your parent/legal guardian choose how identifiable content featuring you may be used by The BUSY School Calgary and THE BUSY GROUP LTD.

Permission is voluntary. You can say no to any option, and you can also choose not to take part in a particular photo, video or interview. Saying no will not affect your enrolment, education, School services or ordinary School activities.

Before you submit

You can decline any particular photo, filming or interview activity, even after this form is submitted.
Authorised photographers, videographers, agencies and technology providers may create, edit, store or distribute approved content on behalf of the School or Group. They do not receive independent reuse rights. Content may be cropped, edited, shortened, captioned or formatted, but will not knowingly be presented in a way that materially misrepresents the student.
Online content can be copied or shared by third parties, and authorised systems or providers may store or access content outside Alberta or Canada. If content is supplied to independent news media, the School and Group may not control later editorial use, archiving or republication.
Permission can be withdrawn at any time by contacting [PRIVACY CONTACT / EMAIL]. Reasonable steps will be taken to stop new use and disclosure and, where practicable, remove content from channels under direct control. Withdrawal cannot guarantee removal of material already printed, distributed, published by news media, downloaded, copied or shared by others.

Consent period: Permission takes effect when this form is submitted and expires 24 months after the student leaves The BUSY School Calgary, unless withdrawn earlier.

Privacy notice

The BUSY School Calgary collects this form and associated identifiable media under section 4(c) of Alberta’s Protection of Privacy Act (POPA) for School communications and administration of media permissions. Approved content will only be used or disclosed as authorised by POPA, including consented uses under section 12(1)(b) and disclosures under section 13(1)(c).
